Parental Agreements

Medical Release

I give my consent and authority for the staff of Edmonds Unitarian Universalist Congregation (EUUC) or designated adults to take action to help ensure the safety, health and welfare of my children, youth, and wards. I also request and empower EUUC staff or designated adults to authorize medical personnel and hospitals to provide all medical care, including but not limited to hospital tests, emergency surgical care, pathology, radiology and anesthesia, surgery and prescriptive drugs for the health of my child.

Classroom Field Trips

I give permission for my children and youth to participate in Sunday morning walking field trips as part of the Faith Development program. This includes nature walks to nearby parks, and, for high school youth, visits to nearby coffee shops and restaurants. (No vehicular travel is covered by this agreement; such field trips will require additional signed forms.)

Law Violations

I understand that if one of my children, youth, or wards break any federal, state, or local laws, that child or youth will be asked to leave the event, and I will be asked to pick up my child, youth or ward. The event might be a Sunday morning class or any group, activity or meeting at EUUC or as part of a field trip, conference, or other event held away from our building and property.

Up-to-date Information

I will notify the Families and Faith program if there are changes to the information — including medical, insurance, and contact data — that I provided to EUUC so that the adults who work with my children, youth, and wards have current information.

Legal

On behalf of my children, youth, and wards, I release, discharge and covenant to hold harmless the Edmonds Unitarian Universalist Congregation, its officers, employees and volunteers, from any and all claims, causes of action, and liability of any kind or nature, including personal injuries or death, or in any way arising out of, directly or indirectly, the children’s, youth’s, and wards’ attendance or participation with EUUC.
